Gregory Fay Kozak

Gregory Fay Kozak, September 6, 1948 - April 23, 2026

Gregory Kozak passed away suddenly at home on April 23, 2026 at the age of 77. He was born on September 6, 1948 in Brigham City, Utah to Nicholas Phillip and Pollyanna Scholty Kozak.  He lived in Brigham City his entire life, graduating from Box Elder High and also attended Weber State.

Greg served for 27 years for the Brigham City Police Department. After retiring from BCPD he work for 18 years as custodian for Box Elder School District. 

He married Linda Lewis and together they had 2 children, Phillip Dean and Paula Kim.  They were later divorced. 

He then met the love of his life, Kayleen Braisford.  They were married on May 22, 1993.  On February 27, 2009 they were sealed in the Ogden Temple.

He is preceded in death by his parents, sister Rickie Tea, a granddaughter, great granddaughter, and his best and loyal friend Lucy Lou.

He is survived by his wife Kayleen, Son, Daughter, 12 Grandchildren and 19 Great Grandchildren.

Viewing will be held on April 29, 2026 at the Brigham City 4th ward from 9:30-10:30 followed by a graveside service at Brigham City Cemetery at 11:00am. Thank you to the 4th ward compassionate service for Luncheon.
